Learn how to use Smart Blocks in the Konfidens form builder to collect client details once and have them automatically saved to the client's profile — no manual copy-pasting required.
❗ Smart Blocks are available inside any form you build with the Forms add-on. If you don't see the Forms module, ask a practice admin to enable it under Clinic functionality first.
1. Open the Smart Blocks section in the form builder
While editing a form, look for the Smart Blocks category alongside your regular input blocks. Unlike standard text or multiple-choice fields, Smart Blocks are specifically designed to write to the client's stored profile data.
2. Add a Personal Details block and choose the field
Drag the Personal Details smart block into your form canvas. A dropdown will appear on the right where you can pick exactly which structured client field this block should represent — for example address, next of kin, or work status. You can drag in the same Personal Details block multiple times to collect several different fields on one form.
3. Customize the question label, placeholder, and help text
Once a Smart Block is in place, you can still tailor how the question reads for the client. Under the Label input, replace the default field name with more natural wording — for example, changing "Work status" to "How is your current employment situation?" You can also add placeholder text, which appears as a light hint inside the input and disappears once the client starts typing, and help text, which displays underneath the input for any secondary context you want to give.
4. Let client responses sync automatically to the profile
Once the client fills out the form and submits it, the values they enter through Smart Blocks are written directly into the matching fields on their client profile — there's no manual data entry needed on your end. This is especially useful for pre-session intake, where you'd otherwise have to chase and re-key client details.
