This article walks you through creating a new form in Konfidens, dragging in input blocks, and configuring the basic field types you'll use most often.
❗ Forms is an add-on feature. If you don't see Forms in your left-hand sidebar, an admin needs to enable it first under Clinic → Functionality.
1. Create a new form
Click Forms in the left sidebar. The forms list is where every form you create will be shown; it starts out empty for a new practice.
From the Forms section, click the + New Form button to start a new form.
2. Open the canvas and name your form
Give the form a name (for example, "Intake Form") and save it.
3. Add an introductory text block
Start building the form by dragging input blocks from the block panel onto the canvas. A Text block is a good first element — use it to give the form a title and write an introduction that fits your practice. Click the X (or anywhere on the blank page) to close the block editor and add new blocks.
4. Auto-populate client details with Smart Blocks
Konfidens includes Smart Blocks — input types that write directly back to the client's profile instead of just collecting a form reply. Drag in the Personal Details smart block and a dropdown appears on the right, letting you choose which structured client field to collect (name, address, and so on). When the client completes the form, their profile is automatically updated with these values, saving you from manual copy-pasting before a first session.
You aren't limited to one instance of a smart block — drag the same block in again to collect another value, such as address, whether the client has children, or their work status. This lets you build out a full set of structured questions using the same block type repeatedly.
5. Customize field labels, placeholders, and help text
Each field has three separate wording options you can configure:
Label — the actual question text shown for the field. For example, you can change "Work Status" to something more descriptive like "How is your current employment situation?"
Placeholder — a hint that sits inside the empty field and disappears as soon as the client starts typing.
Help text — extra, secondary context that displays underneath the input, useful for optional clarification that shouldn't compete with the main question.
✅ Use the label for the actual question and reserve help text for optional clarifying context — it renders in smaller text below the field so it doesn't compete with the main question.
6. Save your form
Once you're happy with the blocks and wording, click Save. You're taken back to the forms list, where your new form now appears alongside any others you've built.
7. Send the form to a client
Send a form from wherever you're already working: the send icon in the Forms list or the Forms tab in a client's record. Pick the client, send, and they get an email link — no login needed, works on any device.
