Collecting Client Consent and Signatures Through Forms

This article shows how to add a consent block and a signature block to a Konfidens form, so you can capture compliant client sign-off — for contracts, privacy policies, research permissions, and similar agreements — directly through the form-filling flow.

Before you can use the consent block, at least one consent type must already be configured in your practice under Clinic → Consent. This article assumes that setup exists; the consent-configuration process itself is covered in a separate guide.

1. Add the Consent smart block to your form

While editing a form, drag the Consent block into the canvas. This block automatically pulls in the consent types you have configured for your practice.

2. Choose which consent to collect

With the block selected, pick the specific consent type you want this block to represent from the list of consents set up in your practice. You can add multiple consent blocks to a single form — for example, one required block for your client contract and a separate voluntary block for research or supervision consent.

3. Set the consent's visibility

Decide whether the consent should be Visible or Not visible on the form.

  • Visible shows the client an explicit checkbox they must tick to give consent.

  • Not visible assumes the surrounding content of the form itself already communicates the consent, so no checkbox is shown to the client.

⚠️ If you set a consent block to "not visible," the client won't see any confirmation prompt for it — make sure the form's wording already makes clear what they're agreeing to.

4. Mark the consent as required or optional

Toggle whether the consent is required to submit the form. Use "required" for anything that's mandatory before you can proceed with the client, such as a client contract or privacy policy, and leave it optional for voluntary permissions like using their data for research or discussing their case with a supervisor.

5. Add a signature block for contracts

If the form is a contract or another document that needs a formal signature, drag in the Signature block as well. This gives the client a dedicated area to sign off on the form when they fill it out.

Once your consent and signature blocks are configured the way you want, click Save to store the form. It's now ready to send to clients using any of the standard sending methods — from the forms list or from within a client's record.
